In order to strengthen campus epidemic prevention and to comply with the instruction of the Central Epidemic Command Center, students who go to get vaccinated or cannot attend classes due to ” self-quarantine”, ” self-isolation “, and ” self-health management “, may apply for vaccination leave and absence due to COVID-19 Prevention. The descriptions are as follows:

(I) Directions of vaccination leave:

1. Number of days off: up to three days (including the day of vaccination).
   Students who feel unwell after being vaccinated can apply for vaccination leave. If they still feel unwell after the vaccination leave limit is exceeded, they can apply for sick leave according to the school’s student leave rules.
2. Attach supporting documents: COVID-19 vaccination record card.
3. Vaccine leave is not included in the record of absenteeism.
4. Remind students that it is recommended to be accompanied and taken care of during the period to avoid being alone. 

 
(II) Directions of absence due to COVID-19 Prevention :

1. Those who are unable to attend classes due to ” self-quarantine”, ” self-isolation “, and ” self-health management “, COVID-19 test or school-related regulations, etc., and who have relevant official notices, supporting documents from government units, or school notices, may apply for epidemic prevention leave.
2. Epidemic prevention leave is not included in the record of absenteeism.
3. Number of days off: According to the regulations of the Central Epidemic Command Center or NTU Epidemic Prevention Team.
4. Proof documents: attach relevant official notices or supporting documents from government agencies, or school notices.

為強化校園防疫，並配合中央流行疫情指揮中心之決議，同學前往接種疫苗或因「居家隔離」、「居家檢疫」、「自主健康管理」等原因而無法上課者，得申請疫苗假及防疫假。分別說明如下：

一、申請疫苗假說明：

1.  請假天數：最多三天(含接種疫苗當日)。
   同學於接種疫苗後若覺不適者，可申請疫苗假，若超過可請疫苗假期限後，仍感不適就醫或休養期間，可依本校學生請假規則申請病假。
2. 檢附證明文件：COVID-19疫苗接種紀錄卡。
3. 疫苗假不列入出缺席紀錄。
4. 關心提醒同學請假期間最好有人陪伴及照顧，避免獨處。

 
二、申請防疫假說明：

1. 因「居家隔離」、「居家檢疫」、「自主健康管理」、採檢或學校相關規範等必要防疫措施等原因而無法上課，並有政府單位之相關正式通知、證明文件或學校通知者，得申請防疫假。
2. 防疫假不列入出缺席紀錄。
3. 請假天數：依中央流行疫情指揮中心具感染風險民眾追蹤管理機制或學校防疫小組規定。
4. 證明文件：政府單位之相關正式通知、證明文件或學校通知。
